Please complete this application to apply for new service. This service is available for residential and commercial customers requesting water and wastewater services within established areas of First Utility District. If you would like to establish service at a location that does not currently have an existing meter present, such as new construction or development, you will need to visit Customer Service office to make your request.

*Rental agreements and proof of ownership documents are required prior to connection of service, and can be submitted by fax 865.675.4029 or email

Fee Information: All customers requesting service connection are required to pay a $35.00 non-refundable service charge. Rental property tenants are required to pay a $100.00 refundable deposit in addition to the service charge. Pending credit verification, fees are required at setup of service.

By placing “I Agree” in the field area, I understand that all water supplies at my residence must be turned off prior to services being activated. This only applies to service/meters that are currently inactive or winterized. If the meter continues to spin after water is turned on, I understand that First Utility District will disconnect service and require me to schedule a two hour window of availability for a technician to reconnect services while I am present. This requirement is for the protection of my residence and to prevent possible property damage.

By checking this box, I, owner or renter of premises, hereby apply to THE FIRST UTILITY DISTRICT (herein after referred to as the DISTRICT) to supply water and/or sewer service to the above premises, and agree to pay for such services when the statements for same are rendered therefore in accordance with the rates, charges and regulations in effect at the time the service is rendered.

I agree to abide by and be subject to all the Ordinances, Codes and Regulations of the District governing the installation and use of water and sewer services, and agrees that the District may require from me, or tenant of the premises, a cash deposit of such amount as the District may deem adequate Security for payment of services rendered these premises pursuant to this contract.
